"Polly" está basada en la historia real de una menor víctima de una violación en Tacoma, Washington, la niña vio al secuestrador como una persona y se ganó su confianza, aprovechando un descuido de éste para escapar. Kurt leyó la historia en un diario y decidió escribir una canción sobre ella, buscando un efecto impactante al ponerse en el papel del secuestrador.
